Reportable Incidents are specific types of incidents that registered NDIS providers must report to the NDIS Quality and Safeguards Commission. These reports help protect participants and ensure quality of services.
Reportable Incidents include:
- Death of a participant
- Serious injury requiring hospital treatment
- Abuse or neglect of a participant
- Unlawful sexual or physical contact
- Sexual misconduct (including grooming)
- Unauthorized use of restrictive practices
Reporting requirements:
- Initial notification within 24 hours of becoming aware
- Full report within 5 business days
- Documentation of investigation and response actions
- Records must be kept for 7 years
Providers must have incident management systems in place to identify, report, and respond to reportable incidents. Failure to report can result in compliance actions by the NDIS Commission.
